COVID-19 Registration Form

Patient 6 months to 4 years

Patients need 2 doses of the Moderna vaccine to be considered fully vaccinated.

~ Second doses are administered 4 weeks after the first. Appointments will be made upon completion of the first dose.

~ Fully vaccinated patients can receive the 2025-26 seasonal dose. Must have been 2 months since last dose.

Patient 5 to 11 years

Patients 5 to 11 years old will only need 1 dose of the Pfizer vaccine to be considered fully vaccinated.

~ Fully vaccinated patients can receive the 2025-26 seasonal dose. Must have been 2 months since last dose.
